In 2023, the AHRQ announced nine grant awards of $1 million each for up to 5 years to support existing multidisciplinary Long COVID clinics across the country to expand access and comprehensive, coordinated, and person-centered care for people living with Long COVID, particularly underserved, rural, vulnerable, and minority populations that are disproportionately impacted by the effects of Long COVID. The UW Medicine Long COVID Clinic’s COVID Aftercare Recovery and Support (CAReS) Program is one of several initiatives funded by this grant.
The CAReS Program provides additional care coordination and case management support to Long COVID Clinic patients. Our primary goal is to increase accessibility to provide person-centered care coordination. Our care coordination team currently consists of a Patient Care Coordinator, Patient Navigator, and a Nurse Case Manager. Each of our team members serve a unique role that supplements the efforts of other CAReS program team members to provide high quality care support to our patients. Through this coordinated approach, we aim to simplify the healthcare process and provide patients with the guidance and support necessary for their recovery. Our dedicated team works collaboratively to connect patients with the resources and care they need, helping them achieve the best possible outcomes.
